The community team’s main aim is to support patients throughout their illness so that they have the best possible quality of life, relief from symptoms, and flexible choices about where they are looked after.
The team works with GPs, district nurses, hospital consultants, specialist hospital nurses and other health professionals to develop care programmes that meet the individual needs of each of our home-based patients.
Each patient has their own designated community specialist palliative care nurse, to provide highly specialised knowledge, support and advice to them, their family and carers and the other health professionals caring for them. The nurses also give patients and their families emotional and psychological support.
They don't provide intensive home nursing or help with daily physical care. But if a patient needs this level of nursing, they can help to organise it by contacting the district nurse and Sheffield’s city-wide intensive home nursing service.
Our community nurses can also give home-based patients access to other services of ours, according to their needs.
