We are looking to recruit an enthusiastic, experienced and motivated Triage Nurse to join our community team. The successful applicants will be joining an innovative and forward thinking team, contributing to the delivery of high standards of clinical care across the community of Sheffield.
This successful post holder will be responsible for triaging phone calls and new referrals that come into the Specialist Community Palliative Care Service at St Luke’s Hospice. They will work closely with the specialist community palliative care team and primary care services ensuring that patient receiving palliative care at home receives an integrated service in a timely manner. They will work under the direct clinical leadership of advanced practitioner level community nurses but will be responsible to the Lead Nurse for Community Services.
Applicants for these posts will be first level registered nurses with significant post-registration experience. You will be motivated, committed and have a flexible approach with a passion to progress.
