Our
Hospice at Home team took on 920 new patients last year and
they made 7,355 home visits.
St
Luke's Hospice at Home Team is made up of specialist nurses,
physiotherapists, social work assistants and an art therapist.
They draw on the hospice's full range of professional and
technical services to complement their own high level of expertise
and provide home-based support to the patient and their family
for the duration of the illness.
The
specialist nurses liaise closely with community nurses, GPs
and other members of the primary health care team. There is
an advice and support service for patients and their families
to allow patients to stay at home, if they prefer.
The
Hospice at Home nurses contribute to training and education
in Nursing Homes with link nurses in the community and Primary
Care Trusts.
There
is a 24-hour help-line, staffed by specialist nurses, for
patients, their families and carers, GPs and other health
professionals in the community, whose worries or concerns
may surface at any time of day or night.
